By completing the following form I agree to permit Ms Sew N' Sew's Quilt Shop to service and repair my sewing machine. I release Ms Sew N' Sew's Quilt Shop from all liabilities. I understand that Ms Sew N' Sew's Quilt Shop will notify me when my machine is ready for pick up and that I have 90 days to pick up my machine unless other written agreement is made. Machines left more than 90 days will be considered abandoned. If I fail to redeem my machine as agreed, I authorize Ms Sew N' Sew's Quilt Shop to sell my machine to cover for their service expenses.
